Making a referral

Anyone can initiate a referral to LTACH. Patients are typically referred to LTACH after receiving care in a hospital – based intensive care unit or a medical surgical unit. Upon approval for admission, a clinical evaluation is conducted by the LTACH team.

Our services are covered in full or in part through Medicare and many health maintenance organizations, insurance companies and state–approved health plans.
