At Mosaic Life Care of St. Joseph - Medical Center, our first priority is you. We are looking forward to serving you. We understand that you may have some questions and concerns when you are going to be a patient. Below is some useful information you may need for your visit. Medical Records
Copies of your medical records can be sent to your physician or to another hospital without charge. Written consent and a small fee is required to release copies of medical records to patients, and we ask your understanding in allowing time for the records to be completed. Please call (816) 271-6080, 8 a.m. - 5 p.m., Monday - Friday, closed Saturday, Sunday and Holidays.
Admission and Room Assignments:
When you enter the hospital, you will go through the admissions process where you will be assigned a room. During your admission, you will be asked to sign an authorization form for medical and/or surgical treatment. Parents or legal guardians must sign for minors.
Valuables:
For your protection, all jewelry and money (other than small change) should be left at home. Items such as eyeglasses, contact lenses, dentures and hearing aids should be stored in a proper container when not in use. If needed, the Security Department has a safe available for safekeeping of your valuables.
Personal Items:
To make your stay more comfortable, we recommend that you bring a few personal items, such as a toothbrush and other toiletry articles, bathrobe, slippers and a nightgown or pajamas.
Medications:
Your doctor will prescribe the drugs you may need during your hospital stay. Please bring all of your medications from home. A member of the pharmacy staff will review your medications. The staff member will need to know how often you take your medications.
340B Program
The 340B program was created in 1992 to expand access to affordable medications to low–income populations. It allows hospitals to purchase prescription drugs at a substantially lower price, passing that savings along to low–income patients who can receive prescriptions at a discounted price. You have the right to choose the pharmacy of your choice.
To be an eligible patient of our 340B program a patient must:
- See a provider that is either employed with, contracted with or referred from Mosaic Life Care AND
- Mosaic Life Care must have record of care (i.e. medical record) AND
- The prescriber must be in Mosaic Life Care St. Joseph - Medical Center or a facility (i.e. child site) that is 340B eligible.
If you wish to transfer any of your prescriptions to a 340B participating pharmacy, please contact your Mosaic Life Care provider.
Telephone:
Your room is equipped with a telephone for your use and convenience. You do not incur any additional charges by using this phone for local calls. Your incoming calls may be dialed directly to your room. Please remember to furnish your room phone number to relatives and friends.
Television:
For your enjoyment, your hospital room is equipped with a color television that can be operated from your bed. There is no additional charge for use of the television and the local cable service.
Smoking:
The use of tobacco in any form is not permitted at Mosaic Life Care.
Flowers and Mail:
Flowers, packages and mail are delivered to patient rooms daily. Flowers are not permitted in the Critical Care, Surgery or Post Anesthesia Care units. Mail received after your discharge will be forwarded to your home. You can also give outgoing mail to a member of the nursing staff.
Friends and family can purchase flowers, balloons and gifts at our Gift Shop. Free email messages are also delivered to patient rooms by our Volunteers.
Patient Relations:
A Patient Relations Coordinator is available Monday-Friday from 8 a.m. – 4:30 p.m. They can be reached at (816) 271-1215. If a Patient Relations Coordinator is not immediately available, please leave a message and they will return messages within 24 hours. If you have concerns that need to be addressed during times when the office is not open, please leave a message and a House Supervisor will be contacted.
Spiritual Care
Chaplain services are available to patients of all faiths; call (816) 271-6040 or ask your nurse for assistance. An interfaith chapel for patients and visitors is located on the first floor of Mosaic Life Care at St. Joseph - Medical Center across from the Spiritual Health office.
Lost and Found
Personal items found within the medical center are kept for 30 days in the Patient Security Office, which can be reached at (816) 271-6044. After that time the items are kept in the Patient Relations Department, which can be reached at (816) 271-1215.
