Your Home Health Team
A Home Health team may consist of the following individuals depending on your needs, situation and doctor’s instructions.
- Registered nurses (RN) – provide a variety of in-home medical services for patients unable to care for themselves or need help monitoring conditions.
- Case managers – may be an RN, but is the person who has primary responsibility for the team involved in a patient’s home health care.
- Licensed practical nurses (LPN) – work with patients on a more regular basis and report to the RN or doctor on the progress of care being administered.
- Nursing assistants – provide personal care support including taking vital signs, making beds, etc., but can only conduct medical services while supervised by an RN or LPN.
- Medical social workers – work with patients and their families to educate them on what services are needed and help remove any barriers to the patient receiving home health care. Medical social workers can help coordinate medical equipment rental and arrange transportation for follow-up visits.
- Physical therapists – provide home health physical therapy which is focused on helping a recently discharged patient with simple exercises to get them moving and help restore a range of motion.
- Physical therapy assistants – works one-on-one with patients and aids the physical therapist in helping patients regain a range of motion.
- Occupational therapists – use everyday activities to treat patients and help them develop, regain or improve daily living skills.
- Certified occupational therapy assistant – provide support to the occupational therapist.
- Speech therapist – assesses, educates and treats patients suffering from speech and voice disorders, swallowing dysfunction and cognitive issues in order to help them regain their ability to produce and understand speech.
- Registered dietitian – creates and puts into action a nutritional plan based on a patient’s condition and nutritional needs as determined by the doctor and input from the other members of the home health team.
- Pharmacy support staff – works with Home Care Services nurses and therapists to provide infusions and specialty products to patients at home.
- Office support staff – schedules visits and follow-ups with patients and their families and coordinates required paperwork with acute care providers.
