Insurance companies may offer a number of different plans. It is important that you contact your insurance provider to see if Mosaic Life Care is considered an in-network provider.
If you have multiple insurance plans, it’s important to determine your benefits from each plan. To start, you need to find out which plan is your primary insurance provider, which you can do by contacting your insurance company or asking your employee benefits staff. You will also need to contact your primary insurance provider to find out if Mosaic Life Care is in network.
Questions to Ask Your Insurance Company
The following are a list of questions that you may find helpful when talking to your insurance provider about coverage options. For reference, be sure to write down the name of the person you spoke with and date that you spoke with them.
- What type of insurance plan do I have?
- Is Mosaic Life Care in my network? If they say yes, you can then ask:
- Does my insurance plan cover physician costs, as well as in-patient and out-patient hospital services?
- What, if any, are my co-payments?
- If Mosaic Life Care is not in my network, what are my out-of-network benefits?
- If I choose to go out of network, what do I have to pay? How much is my deductible?
- If I go out-of-network, what percentage of my bill will be paid by the insurance company?
- Does my insurance plan offer any additional coverage through special networks for treating medical conditions such as cancer?
- Does my policy have an out-of-pocket maximum?
- If my out-of-pocket expenses reach a certain amount, will the insurance carrier ever reimburse at 100 percent?
- If so, does this mean that my claims will then be paid in full, or will I still be responsible for the difference between Mosaic Life Care’s charges and usual, customary and reasonable (UCR) rates?
- Before I see a doctor at Mosaic Life Care, do I need a referral from my primary care provider, or will I need authorization from my insurance carrier?
- Will I be covered for any testing, pathology or radiology charges that may be incurred as a part of my initial consultation with a Mosaic Life Care doctor?
