Personally owned Phone/Tablet

Acceptable Use Agreement

This document specifies requirements for use of personally owned cell phones/tablets for access to Mosaic Life Care’s network and/or email.  It is essential to provide adequate security and integrity of Mosaic Life Care’s data, servers and other technology infrastructure assets.  Caregivers must agree to the terms and conditions set forth in this agreement to connect mobile phones/tablets to Mosaic Life Care’s network and/or email.

 Acceptable Use

  • Mosaic Life Care defines acceptable business use as activities that directly or indirectly support the business of Mosaic Life Care, as defined in HR2135, Cellphone Acquisition and Utilization criteria and HR2140, Telephone and Mobile Device Usage.

 Devices and Support

  • Smartphones and tablets capable of meeting the requirements outlined in the Security section are allowed.
  • Connectivity and access issues with Mosaic Life Care’s network infrastructure and email are supported by Information Technology.  For personal phone/tablets, caregivers should contact the device manufacturer or their carrier for operating system, carrier connectivity, or hardware-related issues.

 Security

  • Phones/tablets that access and store Mosaic Life Care’s data must be encrypted. 
    • In order to prevent unauthorized access, phones/tablets must be password protected using the features of the device and a strong password is required to access the company network.
    • The device must lock itself with a password or PIN if it’s idle for two minutes.
    • Rooted (Android) or Jailbroken (iOS) devices are strictly forbidden.
    • Caregiver’s access to company data is limited based on user profiles defined by Mosaic Life Care Corporate Compliance and automatically enforced.
    • The device may be remotely wiped if conditions below are met:  

1. The device is lost.

2. The caregiver terminates his or her employment or has their employment terminated and have not made good faith efforts to have email removed by Information Technology

3. The Data Security Team and/or Information Technology detect a data or policy breach, a virus or similar threat to the security of the company’s data and technology infrastructure.  In this event, the device will be quarantined and Information Technology will contact the caregiver in an attempt to remove email from the device. A selective wipe (remove only email contents) will be attempted.  At the organization’s discretion, a full wipe (will put back to factory default) will be initiated. The severity and intent of the threat will determine the grace period before Information Technology wipes the device(s).

Risks/Liabilities/Disclaimers

  • The caregiver is responsible for backing up all personal data and content on their phone/tablet.  In the event of a remote wipe, ALL DATA (including purchased content and applications) is removed, resetting the phone/tablet to factory defaults.
  • Mosaic Life Care reserves the right to disconnect devices or disable services without notification.
  • Lost or stolen devices must be reported to Information Technology within 24 hours, or immediately if clearly stolen. Caregivers are responsible for notifying their mobile carrier immediately upon loss of a phone/tablet.
  • The caregiver is expected to use his or her phone/tablet in an ethical manner at all times.
  • For personally owned phones/tablets, the caregiver is liable for all costs associated with his or her phone/tablet.
  • The caregiver assumes full liability for risks including, but not limited to, the partial or complete loss of company and personal data due to an operating system crash, errors, bugs, viruses, malware, and/or other software or hardware failures, or programming errors that render the phone/tablet unusable.

Mosaic Life Care Life Care reserves the right to take appropriate corrective action up to and including separation for noncompliance with this policy.
